Perthe Disease

Perthe’s disease is the syndrome in which an avascular event affects the capital epiphysis (head) of the femur.

Following avascular event, growth of ossific nucleus stops & the bone becomes dense.

The dense bone is subsequently resorbed and replaced by new bone, during which process the mechanical properties of the femoral head are altered such that head tends to flatten & enlarge.

Types of Dysplasia Hip / Congenital Dislocation of Hip

The patients most often present with a limp.
The limp is exacerbated by strenuous physical activities & alleviated with rest.
The second most frequent complaint is pain, which may be located in groin, anterior hip region or laterally around the greater trochanter.
The two primary means of treating symptoms related to this disease are bedrest & traction.
The beneficial effects of bedrest are greatest around the time of development of subchondral fracture.

Treatment

Long Term Prognosis: The most important predictor of long term outcome of the disease is the shape of the healed femoral head & its congruency with acetabulum.

Long term outcome is directly related to the shape of the healed femoral head & its congruency with acetabulum, the emphasis is managing the patients with this disease is careful containment of the head so that it heals properly.

If conservative treatment fails to achieve this goal, surgical interventions should be considered.
